The square shaft is confusing me. I originally thought it was a first gen 12/0 Senator spool

To be sure is it a Penn spool? It looks odd.

I am pretty sure that's not a penn spool.  Not sure if it's an ocean city, LA coxe, or something else.  Someone should be able to solve the mystery.

Here is an Ocean City Panama, 12/0 spool still installed.  Looks to be a match, but I'd have to take the reel apart to be sure.

Yeah that looks pretty darn close. Is that your reel?

No worries.

I think John Elder confirmed it to be a spool from an OC 12/0 Balboa or Panama.  I wonder what became of the rest of the reel.  Maybe the owner upgraded to a 1 piece from a more modern OC in order to fish it with mono?

John is correct. Here is an Ocean City Balboa spool, head plate side.
