If Alan can't supply 4 for you then I can. Send me a PM when you get ready. I sold a bunch of them on ebay and when it slowed down I pulled the ad and stashed my parts. I may have about 16 of them. I did not tap them to 5-40 and left them at 4-40. This way you get a thicker wall and they will last longer.

you're missing one, right?  if so, i've got you covered.  but if you still have inserts that work, then put a light coat of grease on them and stick with them.  i only have dozen left and they have to last for the next 20 years.  if you need to replace any that are missing, just send me your address and i will drop it into the mail, no charge.
